•I understand that a mild-severe allergic reaction is possible with all services and my esthetician is not at fault if this does occur. 
•I agree to contact JJ Cosmetics & Spa directly if any irritation or extreme fall out occurs within 48 hours after lash appointment or I will be financially responsible to resolve issue.
•I agree to allowing my esthetician to take photos of finished service for potential advertising purposes.
•All sale are final/No refund
•A $45.00 late fee will be charged after 15 Minutes late, Refused to pay appointments will be canceled.
